In this case you must provide the GAME.DAT file.
If it's very big you can use the following script and uploading only the two generated files:

Please post a screenshot of the game folder.
I'm sure there is another smaller file there like GAME.idx or similar because that GAME.DAT is just a container without table.

nope only GAME.DAT and INFO.DAT maybe INFO contains the table

It looks like a slightly modified version of the format covered by http://aluigi.org/bms/arika.bms
Spent some time on it but didn't find any clue because the decryption is not correct.

The same person who cracked Mario Tennis (2017+)'s compression made this https://github.com/simontime/ArikaTools/ to extract content from Arika's Dr. Mario titles. Currently the only tool present is unmscf, to extract the audio streams. There's no builds for it so I'll provide one.
